This FREE in-person event will present topics including:
Attendees will be able to demonstrate knowledge of pathophysiology of disease, interventions, POLST forms and goals of care, safe transitions between hospitals and post-acute facilities, and use of respiratory equipment, all aimed at improving health of individuals in long-term care as demonstrated by nursing participants obtaining a post-test score of >=75%.
Target Audience: Nursing home staff and clinicians including RNs, LPS, social workers, administrators, and ancillary staff interestd and involved in process and quality improvement.
Presented by: Integrated Care Department at Atlantic Health System
Credits: 5.75 NJSNA CEUs will be awarded to those who successfully complete all requirements. New Jersey social workers will receive 4.5 CE credits.
*Note: Attendees seeking CEUs must be present for the entirety of the program.
Parking: Attendees traveling to Morristown Medical Center for the conference will be required to park offsite on South Street with shuttle service directly to the Medical Center. Additional specifics will be sent to registrants in advance of the event.
