This FREE in-person event will present topics including:
- Goals of care
- Heart Failure
- Chronic Obstructive Pulmonary Disease
- Pneumonia
- Diabetes
- Hospital/SNF transitions
- Behavioral Health interventions
Attendees will be able to demonstrate knowledge of pathophysiology of disease, interventions, POLST forms and goals of care, safe transitions between hospitals and post-acute facilities, and use of respiratory equipment, all aimed at improving health of individuals in long-term care as demonstrated by nursing participants obtaining a post-test score of >=75%.
Target Audience: Nursing home staff and clinicians including RNs, LPS, social workers, administrators, and ancillary staff interestd and involved in process and quality improvement.

Credits: 5.75 NJSNA CEUs will be awarded to those who successfully complete all requirements. New Jersey social workers will receive 4.5 CE credits.
